  • Eventually, the car of the train starts changing, with bloodstains and splatter appearing all over the interior, with the impatient form having the word murder written all over, tormenting Michael even more on how he killed his wife and now his lack of action led to the death of so many.
  • After some more loops, Michael comes across the same car filled with people having their faces censored and a child holding the hand of her father, all having a place to go and a family to care for.
  • The announcement speaker then starts again, calling Michael a murderer over and over again, further causing more distress and trauma on Michael, whose condition becomes even worse.

  • This proves that Michael was treated for his mental health and had a regular therapist who seems to work for him very well but at some point for some reason his mental health severely deteriorated and he completely stopped seeing his therapist and even responding.
  • This leaves the therapist baffled trying her best to get in touch with Michael again to no avail.
  • As Michael continues down the cars, the embodiment of his guilt shown through reimagined humans all stare at him with one even holding a photograph with it showing how the blood of all the innocent people on the train is on him when this human disappears with a demon appearing in its place for a split second before vanishing.
  • It seems as if Michael was doing well and slowly coming to terms with his mistake leading to the massacre of people on the train but some sort of paranormal demonic entity hunted him and constantly tormented him with vivid imagery of the day of the massacre and the dead which slowly deteriorated his mental health leading to his death.
  • Eventually, Michael arrives in a car which reports on the day of the massacre, explaining Michael in fact was the tactical team leader who made the misjudgment in his plan to free and save the hostages which led to all of them, all fifteen of them, dying by the ruthless terrorists.

  • Remembering everything vividly now, after what seems to have been amnesia, Michael feels extreme claustrophobia, being trapped in the same car that started it all, what fueled his trauma and mental breakdown.

  • Further down the train, the ever changing train shows a caged entity whom Michael has prompted to free, which could be the representation of his wife on how she suffered under his hands, even being trapped now in a deformed and demonic form.
  • As Michael manages to free her, a large demonic entity chases after him, which then transports him into a different world, where he witnesses a floating entity with feathers and eyes,
  • which resembles a biblically accurate angel, which might act as a figure for judgment or companionship, whether helping Michael to go through his next passage to the other world or to be judged for the things that he did in the last world.
  • Therefore, Michael seems to have been in purgatory, being punished for his misdeeds and sins, having a personally designed punishment to suffer the most amount possible.
  • He tormented himself in the real world and blamed himself for the death of the victims of the train hostage situation and he could never move on.
  • Hence why in this world he is suffering the same way but in here it is extended to a higher level, being more vivid and real, essentially his nightmares coming to life.
